  • Safeguards: We perform annual HIPAA risk assessments, train our workforce on privacy/security, and enforce sanctions for violations.

9.2 Retention & Destruction of PHI

  • Retention Period: We retain PHI for at least six (6) years from creation or last use—or longer if required by state law.
  • Secure Destruction: After retention expires, we destroy PHI using NIST‑compliant de‑identification or physical shredding.

9.3 Breach Notification

If there is an unauthorized disclosure of unsecured PHI, we will:

  • Notify affected individuals within 60 days of discovery.
  • Notify the U.S. Department of Health & Human Services as required by the HITECH Act.
  • Mitigate any harm and implement measures to prevent recurrence.
